Tirzepatide Weight Loss: What the Studies Found

Tirzepatide weight loss varied from person to person

Your belt may loosen before other people notice. Tirzepatide weight loss was large in key studies, but the figures are group averages.

The biggest study followed 2,539 adults with obesity. None had diabetes, and average loss at the top dose reached about 21% over 72 weeks.

People taking the sugar pill lost about 3%. Another Phase 3 test near approval found 47% more loss than with semaglutide, which copies 1 gut hormone.

You can't know beforehand whether blood pressure, cholesterol, or sleep apnea will improve. Those gains may not happen for you.

Time isn't the only catch. Stomach and bowel trouble often appeared while doses went up.

We took every figure from a published study. We can't forecast your scale or promise a result.

Tirzepatide weight loss was large in study 1

This was study 1 in the weight-test series. It was a large Phase 3 test, but it left out people with diabetes.

The 2,539 adults had obesity, or excess weight with another health problem. A body-size score of 30 meant obesity; 27 meant excess weight plus another health problem.

People received 5, 10, or 15 mg of tirzepatide, or a sugar pill. Doses rose over a 20-week period.

The study lasted 72 weeks. At week 72, average weight loss was [4]:

  • 5 mg: 15.0%, against 3.1% with the sugar pill.
  • 10 mg: 19.5%, against 3.1%.
  • 15 mg: 20.9%, against 3.1%.

The main problems were nausea, vomiting, diarrhea, and constipation [4]. Most weren't severe, and they appeared mainly while doses rose.

These findings led to weight approval in November 2023. They still can't tell you the number you'll see on your scale.

Study 5 found more Tirzepatide weight loss than semaglutide

This was study 5 in the weight-test series, and it lasted only 72 weeks. The Phase 3 comparison followed 751 adults without diabetes.

It compared tirzepatide with semaglutide, a shot that copies 1 gut hormone called GLP-1. People stayed on the strongest amount they could handle.

That was 10 or 15 mg for tirzepatide and 1.7 or 2.4 mg for semaglutide. At week 72, researchers allowed for small differences between the groups [5]:

  • Tirzepatide: average loss was 20.2%.
  • Semaglutide: average loss was 13.7%.
  • The gap: 6.5 points on the percent scale. Chance probably wasn't the cause.

More people taking tirzepatide passed the 10%, 15%, 20%, and 25% marks [5]. They also lost more around the waist.

A second study in type 2 diabetes also favored tirzepatide. If you have type 2 diabetes, you can't assume this result will fit you the same way.

Tirzepatide vs semaglutide favored Tirzepatide for weight

The direct studies favor tirzepatide for weight and sugar. They don't settle the heart question.

One type 2 diabetes study lasted 40 weeks. Tirzepatide lowered the three-month sugar test more than semaglutide, a GLP-1 drug [3].

Tirzepatide also led to greater weight loss [3]. The size of the gap changed with the dose and the illness.

In study 5, run for 72 weeks, tirzepatide again beat semaglutide [5]. Both groups had obesity without diabetes.

A 2026 review used five US insurance studies of people with type 2 diabetes. Its 95% range showed how far the heart answer might move [14].

That range included equal heart risk. The GLP-1 comparison didn't produce a clear heart winner.

Weight loss came with other health changes

Weight gets attention first. Studies also checked blood pressure, blood fats, breathing, liver, heart, and kidneys.

One review found that the upper blood-pressure reading fell at 5 mg, 10 mg, and 15 mg [10]. The lower reading fell too.

Another review also found helpful changes [8]. Harmful cholesterol and blood fats fell, while helpful cholesterol rose.

A later study found 16% fewer combined heart-and-kidney problems than with dulaglutide, a GLP-1 shot [13]. That count joined several kinds of trouble into one result.

Everyone in the study had heart disease and type 2 diabetes. It can't prove the same benefit for you.

In a 2024 sleep study, breathing pauses fell in adults with obesity [15]. That finding backed approval for moderate-to-severe sleep apnea.

A different 2024 study found less liver damage and scarring [12]. A 2025 heart-failure study found fewer symptoms and fewer limits on daily tasks [16].

Each group had a different illness. One good finding doesn't mean tirzepatide treats every health problem.

A weight stall can happen while Tirzepatide still works

A flat scale can happen while the drug still works. In study 1, most loss came during the first 36-52 weeks, then slowed.

Your body may burn less energy after weight falls. Hunger can also return, and one dose won't fit every person.

Study 4 tested people after 36 weeks on tirzepatide. In 2024, those moved to a sugar pill regained about 11.8% of starting weight [18].

People who stayed on tirzepatide kept losing on average. That finding isn't an order to change your dose.
